Vzor žádosti o rodičovskou dovolenou
Žádost je adresována zaměstnavateli a musí obsahovat datum, místo a podpis žadatele. Dne 20. srpna 2019 jsem nastoupila na mateřskou dovolenou. Dne 3. října 2019 se mi narodil syn Jan. Dne 4. března 2020 mi skončila mateřská dovolená. Ode dne 5. března 2020 do dne 11. března 2020 čerpám dovolenou. Žádám o čerpání rodičovské dovolené od 12. března 2020 do 3. října 2020. Žádost se podává před nástupem na rodičovskou dovolenou, v našem případě před 12. březnem 2020.
Take bych se chtela zeptat,kdy je nutne zamestnavateli oznamit narozeni ditete?
Zamestnavatel chce byt informovan po narozeni ditete a preje si zaslat rodny list ditete (ucetni oddeleni).
